The Clarity Health mark — a gradient C with a horizon line and a burst of light, representing bringing the unseen hours of care to light.

Care intelligence that starts with caregivers.

Clarity captures what happens between encounters — the daily care context — and turns it into intelligence care teams can see and act on.

Healthcare organizes itself around the encounter. Health is shaped by everything in between.

Health is lived in the 23 hours between encounters. Days or weeks may pass between visits. Caregivers see it all. Care teams see none of it — until it's often too late.

The evidence

80–90% of what shapes health outcomes never enters the clinical record.

Research consistently shows that medical care accounts for only a fraction of what shapes health. An estimated 80–90% of modifiable contributors to health outcomes are driven by what happens outside clinical care — in daily life, home routines, behaviors, social context, environment, and support systems.

Clarity Health is built to make that missing context visible and actionable — capturing the 80–90% of health context that lives outside the encounter.

Meet Clare

AI that becomes a care partner

Clare is the AI care partner inside Clarity. Unlike generic chatbots or single-purpose healthcare AI tools, Clare understands the ongoing context of care — the daily observations, changes, questions, tasks, medications, appointments, documents, and concerns that shape a person's health between clinical encounters.

See how Clare works
Care happens at home

The most important hours of care leave no record. Until now.

Clarity puts the family caregiver — the person who's actually there — at the center of the health picture, and turns what they see into intelligence the care team can use.

When professionals work with caregivers with clarity, consistency, and communication, caregivers are less stressed — and stop having to fight to be believed.

See care clearly.

Let's talk about what Clarity Health can surface for your patients, your members, or your workforce.

The platform

The home, finally visible to the care team.

Clarity Health turns everyday care context — observations, daily activity, and home-based patterns — into structured intelligence that supports clinical workflows, care coordination, and better decisions.

The data flow

Observation in. Clinical signal out.

Caregiver observes

Clare helps caregivers capture the real-world details healthcare often misses: subtle changes, medication concerns, functional shifts, symptoms, routines, and follow-up gaps.

Clarity structures

Clarity transforms caregiver observations into organized, person-specific patterns anchored to a longitudinal picture of the person's own baseline, daily context and care journey.

Care team receives

Care teams receive the missing between-encounter data — health status and functional changes, caregiver observations, emerging risks, medication adherence concerns — delivered into their workflow for timely action.

Clare · the AI care partner inside Clarity

AI that becomes a care partner

Clare helps caregivers capture what they see, organize what matters, notice meaningful changes, prepare for conversations with providers, and turn fragmented information into clearer next steps.

For caregivers

Less weight to carry alone

For caregivers, Clare reduces the burden of carrying every detail alone — tracking the observations, tasks, medications, appointments, documents, and concerns that shape a person's health between clinical encounters.

For healthcare organizations

Real-world context, made actionable

For healthcare organizations, Clare helps transform real-world care context into structured, actionable intelligence that can support clinical workflows, care coordination, risk identification, and more informed decision-making.

Clare does not replace clinicians or caregivers. Clare strengthens them — by helping make the invisible work of care visible.

The Clarity caregiver app home screen: who you care for, this week's appointments, refills and tasks, a Clare Insight noting an evening cough trend, reminders and alerts, and care focus areas.
For the caregiver

One calm place to carry the details — with Clare by your side.

Clarity helps caregivers manage the full picture of a loved one's health: medications, appointments, observations, trends, reminders, tasks, and care updates in one simple app.

Clare acts as an AI care partner, helping caregivers notice changes, prepare for visits, and turn daily care details into insights that can be shared with the care team.

Built for real people under real stress, Clarity is calm, readable, and easy to use — not a clinical portal retrofitted for families.

About Clarity Health

We didn't discover caregivers. We discovered we'd been financing them off the books all along.

Clarity Health exists to make the family caregiver an essential, recognized member of the care team — so people can age at home, safely, with the people who know them best.

Our mission

Making the 23 hours count

Healthcare measures itself in clinical minutes. But the real story of a person's health is written during the 23 hours a day they spend outside any clinical encounter — at home, with family.

We recognize the family caregiver as the source of truth about what happens there. Our platform captures that real-world care context and transforms it into structured intelligence — supporting clinical workflows, care coordination, and better decisions, while integrating caregivers as the essential team members they've always been.

The result: an aging population that can stay home, and a care system that can finally see clearly.

What we believe

Principles that guide the product

The caregiver is the source of truth

The person at home sees what the chart can't. We build around their knowledge, not despite it.

Trusted sources or nothing

Health guidance is grounded in recognized authorities and named plainly — or it isn't offered at all.

Designed for real, stressed humans

Large, readable type. Black text. Plain language. Built for older eyes and harder days.

Signal, not noise

We surface what changed against a person's own baseline — so care teams act on what matters.

The home was free until risk-based payment ended the free ride.
